21st Georgia Unit Forms

   The following forms are for use by unit members. These forms need Microsoft Word®

    to be accessed properly. If you have problems downloading these forms, please

    contact the Public Relations Chairperson at J.r.webb@sbcglobal.net.

 

        Equipment Request Form - This form is used to designate loaned

                    equipment to new members or guests who participate at

                    events. Forms to be submitted one week prior to an event.

 

        Event Sanctioning Request - This form is to be used when 10 or

                    more members attend a non-sanctioned event, and wish

                    to petition the Executive Board for that event to count

                    toward voting rights.

 

        Goober Grabber Paper Request - This form is used to request a

                    paper copy of the Goober Grabber Newsletter.

 

        Health History Questionnaire - This form is mandatory for all

                    members and must be kept on one's person at events.

 

        Needs Assessment - This online form is available for members to

                    submit suggestions to the Executive Board. Submissions

                    may be sent anonymously if desired.

 

        Proxy Vote - This form is used to pass voting privileges to another

                    member if you are unable to attend Remembrance Days.

 

        Unit Dues Calculation - This form is used to calculate unit dues

                    and other optional services. The form automatically

                    allocates funds for insurance, ANV, and unit membership.
